What should you download?
1. If you are a new patient: Please fill out the Initial Health Questionnaire, Personal Information Form, and the Terms and Conditions document. This allows us to better understand your medical history and the reason for your consultation, and to properly prepare for your first visit.
2. SIf Dr. Marc Bony, D.C., orders an imaging test, you can use this request form to have it done at our affiliated facility. X-rays are covered by your health insurance or mutual insurance plan. If not covered, the cost is only €20 at our affiliated facility.
